1. Log in at https://isw.umac.mo/eform/, input your student ID no. and password
2. Click “Select form for submission“, then click “SPC”
3. Choose “SPC Readmission Application 2017/18”
4. Fill in the on-line application form
5. Upon completion, click “Submit”

- E-form system will log out automatically per hour and the entered data will “NOT” be saved.
- Each applicant can only submit the online application for once, please read and fill in the form carefully and completely.
- Please ensure the application information is filled correctly before submission as changes are not allowed once the submission is made.
- Any missing fields, the alert will pop-up at the top-left corner.
- This form serves as an application only, the final arrangement is subjected to the final decision by the college.

F. College Fee Installment Application:
For students who face financial difficulties to settle the college fee may apply for the college fee installment application at the same time. Individual interview with installment applicant is required. Upon approval, the 1st installment will be paid on the original college fee payment period and the remaining amount will be paid according to the designated schedule before the end of the accommodation period.
*Requirement: average monthly per capita household income is lower than the amount as stipulated in Article 3.2.1 of the Application Eligibility of the Guideline for Study Loan Application of Education and Youth Affairs Department
Please approach to office of Shiu Pong College (S9-G016) on/before 10 March 2017 for the College Fee installment application with the following documents:
- Application form (please approach to office of SPC for the related form).
- Explanation letter written by the applicant/family member(s) living with the applicant.
- Family member(s)’ salary proofs issued by the employer(s). For unemployed family member(s), they should submit the declaration of income (please approach to office of SPC for the related form).
- Declaration of household income (please approach to office of SPC for the form).
- ID card copy of the applicant
- ID card(s) copy of all the family member(s) living with the applicant
